Since the first case report on the application of ologen Collagen Matrix for congenital glaucoma at SOE 2013 Congress, Copenhagen (by Dr. Hamdi M), more and more pediatric ophthalmology specialists are paying attention to the potential of the collagen matrix's wound healing effect in eye surgery, previously well-known in adult population and yet to be explored for pediatric ophthalmology. Following the presentation at 2013 WGC, Vancouver, Dr. Nigar Hasanova will present her comparative results at 2013 AAO, New Orleans, demonstrating that ologen CM is a safe and effective wound modulator in combined trabeculotomy-trabeculectomy (CTT) in the treatment of primary congenital glaucoma (PCG): 100% of the CTT cases with ologen CM showed IOP < 18 mmHg at the end of 6 months (mean preop IOP 40.4 ± 6.87 mmHg to postop IOP 13.6 ± 2.01 mmHg; n=10; P < .001) compared with 80% of the cases with CTT alone (mean preop IOP 41.0 ± 8.68 mmHg to postop IOP 16.3 ± 1.89 mmHg; n=10; P < .001). The symposium will cover the topics from the US experience with the collagen matrix (D Godfrey, USA), the combined/ supercharged Ex-Press ologen technique (S Vold, USA), an overview of the biosafe ologen patch graft in bleb revision surgery (R Fellman, USA), to the case reports and review of the anti-scarring solution applied in congenital glaucoma (N Hasanova, Azerbaijan) and non-penetrating deep sclerectomy NPDS (M Moussalli, Argentina).

2013 Annual ASCRS-ASOA Congress (American Society of Cataract & Refractive Surgery) will be held from 4/20 - 4/23 at Moscone Center, San Francisco (AAE booth number 2737). At the congress, our key message is:

“Empower your patient with a choice for healthier blebs”

Patients when receiving glaucoma surgery are often provided with insufficient information about the adjuvants used in the procedures, such as Mitomycin C (MMC) or the alternatives available to date. The barriers preventing optimal patient communication include cost concerns and poor accessibility to the newly established evidence of the alternatives, such as ologen Collagen Matrix.

While remaining common practice in some regions, the application of MMC may sometimes result in thin and avascular blebs: these blebs often require new revision surgery after some time. However, sacrificing a healthier bleb is not really necessary for the success of the glaucoma surgery. There are less- and non chemotoxic alternatives that may help obtain healthy and functional blebs.

To facilitate patient empowerment and communication, surgeons are advised to inform and actively involve their patients in the decision-making of the procedures and adjuvants to be received. Especially regarding the cosmetic effects, the short-term/long-term benefits and risks amid the adjunctive options and combinations, such as MMC, anti-VEGF (Bevacizumab, Pegaptanib) and ologen Collagen Matrix, as part of the evaluation process.

March 21, the first preliminary results of the Aeon Astron Europe B.V. initiated seeding trial entitled: “Trabeculectomy with ologen® Collagen matrix” will be presented by Dr. H.J.M. Beckers from the University Eye Clinic of the Maastricht University Medical Center. This trial, involving two leading Dutch glaucoma centers, the Maastricht University Medical Center and the Rotterdam Eye Hospital, and enrolling a total of 40 patients, is a prospective intervention pilot study to determine the degree of intraocular pressure lowering of trabeculectomy with ologen® Collagen matrix in comparison to trabeculectomy with the antimetabolite mitomycin-C (MMC). Additionally, the safety (per- and postoperative complications) of the two procedures will be compared. It will be the first report on the use of the ologen ®Collagen Matrix in the Netherlands.

The use of ologen Collagen Matrix as an alternative to MMC is an art in glaucoma surgery; it concerns flexibility and suitability of the applications based on patient selection with corresponding surgical adjustments. The debate may touch on the issues involved, including:
- Less/looser scleral flap sutures with ologen CM
- Earlier sutures lysis/release with ologen CM
- Selection: secondary glaucoma, patients with history of failed surgery and risk factors (eg. thick Tenon's capsule)
- The combinative use of ologen CM with low-dose MMC in the difficult cases
